First you need to make your mind clear if you want to achieve something or not. If you clear about your goal and purpose, definately you will study for it,if you are clear about things, you will have response like things in your mind. You will have clutter of thought. If you focus on what to achieve, definitely it will happen in positive way ,so keep yourself focused for your aim.
First of all you have to understand its okay If you are not able to concentrate on your studies, beleive yourself you can do it its completely normal few days before students have anxiety.
you can try these remedies to lessen your anxiety and perform excellent in exam.
-Create a quiet and organized space for studying.
-Fix a time and place for studying and stick to it strictly.
-Make a plan for studies.
-Stay away from distractions.
-Divide your studies into small parts and reward yourself after completing each part.
-While studying, stay away from video games, mobile, TV etc.
-Exercise or meet friends in between studies.
-Get adequate and good sleep.
If you are bored with the routine, then study by making graphs, puzzles etc.
Whatever you read while studying, focus on understanding and remembering it.
Generally, studying for 4 to 6 hours daily can be a good start.
Once you started following your schedule there is no way going back.
