To correct name on your 12th marksheet, you have two options.
1. You may approach to Head Of Board issuing your 12th marksheet. Follow procedure as required. You may be required to provide supporting document which has your correct name such as leaving certificate, copy of birth certificate etc.
2. Through gazette for it you need a notarized affidavit with supporting document. After submitting document gazette office will verify it. Once approved, your name change will be published in gazette.
I suggest you to check if it can be done by option 1 if not then go for option 2.
