Hope you are doing good. Cognizant generally takes interview in 2 steps one is the technical round which is a bit lengthy and the other one is he round . Student need to first clear the apptitude or coding exm to qualify for the technical round which is an elimination round and after that hr round where your are asked to show some of your documents . I think you cleaned the technical round ( hope for best) as they also reject in the he round as well.

This should not be a major problem considering that both the documents will contain other information as well which can be matched and verified.

But in order to avoid any panic or problems at the time of verification you should create a self attested affidavit claiming that both the persons mentioned in the different documents are same. You can supported by using your parents name with proper proofs or any other document if you wish.

Two different dobs which is the right one?

Valid proofs of date of birth for most of the exam forms are 10th marksheet and aadhaar card. Now I hope the dob in your aadhaar card is the right one or if its not then change it. UIDAI has

given direct link — ssup.uidai.gov.in/ssup/login.html for Aadhaar card holders who wish to change their date of birth. Just upload supporting documents and its done!

Changing the dob in 12th mark sheet is comparatively more complicated that entirely depends on your board of education!

Aadhaar card is an accepted valid proof of address and date of birth in India. So make sure to correct any errors in it.

You have to fill it according to your class 10th certificate if even your aadhaar card and passport has your mother's name, Then proceed and fill the NEET form with your mother's name only.

Leave the father's name blank. But if its mandatory to fill it and the form is not getting submitted then it might pose a problem. Let me know if you still have issues.
