Hello Aspirant,
Qualification codes were introduced to classify the different eligibility conditions by the candidates, as there is no upper age limit and also no limit on number of attempts therefore the eligibility range became quite wider and to classify it different codes from 01-07 were introduced.

Practical exams should also be there.
>>> Code 04 :- The candidate can choose this code if he/she has completed pre-professional/pre-medical examination with Physics, Chemistry, Biology/Biotechnology, and English after passing either the Higher Secondary examination or Pre-university or equivalent exam. Practical exams should also be there.
>>> Code 05 :- The candidate who has passed the first year of a bachelor's degree from a recognized university with Physics, Chemistry, Biology/Biotechnology inclusion of practical tests
>>> Code 06 :- Any aspirant who has passed B.Sc. examination with not less than two subjects which includes Biology/Biotechnology, Physics and Chemistry. Apart from this the candidate must have passed 10+2 or it's equivalent with PCB and English as compulsory subjects.

Hi there
In regards to your query according to the latest paper pattern it's not mandatory to have the knowledge of all the four subjects to clear the exam. Although that being said if it's possible having the information about all the four subjects could be beneficial especially considering a particular section of the paper could be of relatively less hardness, which could vital in selection.
Hope this helps, best wishes for future endeavours
