It's normal to be confused at this age but let me make this easy for you.
I will explain you little by little scope and required things for each course.
firstly Engineering is a vast subject it has many branches like chemical, computer, electronic , aeronautical etc.
The basic entrance that is acceptable around the Institution for Engineering is Jee .
As you are pcm student you already as the basic requirement so your need to score 50% in your board's.
The Engineering has a lot of scope and pay is very good .
so you are interested in maths physics or chemistry then hive it a go.
Second would be medical right as you have pcm then this option becomes invalid right.
let's move on
Fashion designer is for those who is interested in clothing , styling , or accessories .
For this you can do fine arts , design etc.
Perk is that there is no age limit for this one .
And the salary range for a little to more as per your experience.
Psychology is not an easy option but if it attract you then nothing is impossible.
You can do bachelor's and then go for doctorate but scope for this course is not much in India.
Though this subject is easy but scope is less .
I have tries my best to explain you about these three but if you want to know more about anything comment below.

Good communication skills:
The entire designing procedure, whether it be jewellery, footwear, clothing or accessories, requires collaboration with colleagues. One can't work on a project alone. So you should be a social person.You should be clear about your own thoughts and you are able to express yourself freely.
Trustworthy and punctual:
You should work according to clients expectations,
If you have given any project then complete it within a specific time period. Understand the taste and preferences of your clients. Be reliable to them.
A Sharp Business Sense:
For surviving in the fashion world, you must have a thorough understanding of business, including the key business areas like finance, sales, and marketing, which are essential to becoming successful in the fashion industry. Creativity is the soul of fashion, but without any understanding of business, it is really hard to profitably sustain a fashion brand.
6. Confidence
Eligibility for becoming Fashion Designer:
Candidates must complete 10+2 or its equivalent from a recognised school board in India.
After the completion of class 12, candidates can prepare for the entrance examinations like NIFT, NID and other entrances for admission in UG & PG design courses.
Entrance exams:
NIFT: It will be conducted once in every year. Candidates will get admission in the university and its affiliated campuses. The 16 affiliated campuses of the NIFT consider the NIFT entrance exam to offer admission.

4. Two PG Diplomas
5. Two Diplomas
6. Two Certificates
That means you can pursue BSc Nursing from Regular Mode with Diploma in Fashion designing from Distance Mode/Open Mode/Online Mode.
Currently Two degree programmes are not allowed to be pursued simultaneously, either from the same University or one from the open university.
But UGC is working on this, may be in future they will allow to pursue Two degree programmes simultaneously.
