If you are looking for Gujarat state counseling then then it won't be a problem, as it is in your native language.
But if you are looking for admission in other states through All India counseling then it may create a problem. General language preferred are Hindi and English in most of the states.

If your EWS category is not valid for central criteria then you can't fill it in your NEET form because it is a nation wide entrance test conducted by NTA. Therefore, state quota is invalid there. So you have fill General Category in your category option. You can fill the counseling form of Gujarat state after the result and fill your state quota there and then it would be taken into consideration for sure.

As you have mentioned that you have ews certificate in gujrati and considering you come under state and not central government so I would recommend you to take out your ews certificate in English even if it comes under state.
Ews certificate eligibility criteria is as follows:-
1-Family income should be less than 8L per annum.
2-One should not own more 1000 sq ft of flat.
3-One should not own more than 5 acres of land.
Where did you see last year Gujarti neet question paper?
It is not possible to have Gujarati in Gujarati only , it is possible that the website from where you downloaded question paper had removed the English part .
This is because,

Without any doubt you can choose Hindi as the medium or the language for your Neet question paper
if you select Hindi as the medium of your neet question paper then you will get your question paper will be bilingual i. e it will be in both Hindi and English .

Hello aspirant, there should be no problem if you have chosen Gujrati language while filling form for JEE mains. JEE mains paper 2021 will be conducted in 12 different languages and during the time of giving exam you will get the option to see the question paper in the language in which you are comfortable. An option will be provided to you during the login time before the exam start to change the question paper language and you can also change the language after the exam has started.
