Internal medicine is a field of practice concentrated on treating adults. Doctors prevent, identify, and treat diseases that affect all adults, from chronically ill patients, to short time care. On an average the salary is around 1.5 lakh per month and varies depending on where you work, how hard you work , years of experience and extra qualification. An experienced MD in internal medicine can earn 4–5 LPM , while a fresher may get 1–1.2 LPM. It is not necessary to do DM after one completes MD, but one can do it if he wants to have more salary.

Thank you for your query. If you want to study in USA you need to first get through IELTS with a score of atleat 7-8 in academic overall. Then you have to pass the medical exam in USA which is required to being qualified to get for a course on Internal medicine.

Yes it is a difficult situation for you to come across. You have great options in both of the field with your rank of 1400. With that said, i i would say that going for MD psychiatry would be a great option at this point of time. Psychiatry is a speciality on its own and you get to deal with every kind of patients and problems in that. There is a extreme shortage of psychiatric and the pay is too good. You can practice in many places like inpatient, outpatient, VA, university counseling centers, emergency rooms, medical wards, integrated into other specialty clinics (i.e. OBGYN, pediatrics) and prisons, to name a few. I hope it helps.
But the last decision will be on what you like better as you will have to go for it for the rest of your life. Choose wisely and best regards
