If you have taken admission in the college allotted to you in AIQ by giving all the documents (if you have taken admissio by completing all the steps) then you will not be able to participate in the state counselling but if you have not reported to the allotted college (if you leave and not complete the full admission procedure) then you may go for the seat you got in the state counselling. Incase you do not join the college given in AIq you will have o forfeit the security deposit but you can continue with the state counselling.
If you have joined the college in AIQ you cannot vacate that seat. However I will advise you to consult your college administration incase you have taken admission in the AIQ allotted college (maybe they will be able to help you out).

With 390 marks in neet you have less than to get private medical college even you belong to BCA catagory. According to previous years cutoff you need to score 400 plus marks for private mbbs seat in Telangana through state quota.

You can participate in AIQ DEEMED/Private institutions counselling to get management quota seats. College tution fee will be very high for this college it will be above 12 lakh annum. Getting a Government Quota seat with 226 marks in NEET is quite difficult as per last year's Cut Off! You can definitely get a Management Quota seat.
But all depends on this year's Cut Off. Cut Offs change every year. And considering your reservation category also in mind we have to wait and look whether you'll be getting or not!

After the first round if you do not want to sustain the seat then you should not fill the form. You can again apply for private MBBS colleges separately in the next round. But remember if you fill the form to sustain your seat in round 1 then you won't be able to participate in round 2 so choose carefully.
