If you really get this score then it will be hard for you to get admission to MBBS or BDS at any top-rated Private medical colleges under Government quota, let alone chances to get seats at Government medical colleges. In other words, getting chance to contest a seat for MBBS/BDS is less even if you participate in All India or State Counselling process.

Last year, the cut off rank for government medical seat under 1G category in Karnataka State Quota was 82617. So based on that it is quite difficult for you to get a government medical college. But keep in mind that cut offs and ranks vary every year depending on the number of seats available, the difficulty level of the paper. Nothing fixed can be said until NEET 2021 results come out.

If you are not a domicile of Karnataka then also you can get seat in KIMS Bangalore.
Here are some eligibility criteria to get admission in Kempegowda Institute of Medical Sciences (KIMS), Bangalore;

Ther is about 3000 medical seats avaialable in Maharashtra alone.therefore there is chance that you will get a mbbs seat 550 marks on the last year cut offs in the collges. As you have mentioned that you got 380 marks in neet UG 2021 and taking into consideration that you want to join this college so let me tell you that the cut off generally stays below 40k for the general category students.
In this marks I don't think that you would be getting a rank of 40k because the cut off is much higher for this rank. Although various factors are considered while deciding the cut offs like number of candidates appearing in the examination, number of vacant seats in college, previous years cut offs.
