For central institutes of Ayurveda, the stipend for both non-clinical and clinical MD seats is the same as the stipend is standardized for all postgraduate MD/MS scholars. So if you are from any branch it makes no difference in the amount you get.

With 22,461 rank in NEET PG (AIQ), getting MD Obstetrics & Gynaecology (OBG) in a government medical college is quite difficult because OBG is one of the most competitive clinical branches. In All India Quota, OBG in government colleges usually closes under 5,000–12,000 rank.
In Telangana State Quota, EWS category may give a little relaxation, but still under 15,000–18,000 rank for OBG in govt colleges.

In Tamil Nadu, for OBC category, MD Anaesthesia in government medical colleges usually closes within 25k–35k rank in NEET PG. With a better rank than this range, you stand a chance.

DNB Medicine is an excellent option too—widely available, equivalent to MD, and feasible at your rank. Keep both state quota MD and DNB Medicine in your preference list for best chances.
