According to your question and taking into consideration that you rank has been changed in the final merit list.
I would like to tell you that the state first releases the provisional merit list because maybe some of the students were ranked wrong even there marks is very good and in the final merit list you can see the change in your rank.

First of all let me make you clear that in All India Quota counselling EWS category is only applicable in Central Universities. So on such marks and category you have very low or almost no chances of getting any government college according to previous year's cut-off. As we go through previous year's cut-off then it shows that if a candidates having EWS reservation scores around 580-590 or more then he/she will have chances of getting seat in any central university.
But yes you can get government colleges under state quota of your state and since you have nit mentioned your state so it's quite difficult to guide you with an accurate information.
Note that the above prediction is made on the basis of previous year's cut-off and the cut-offs are bound to change every year depending upon various factors such as the number of candidates appearing in an examination, the number of candidates qualifying an examination, difficulty level of the paper, and so on.

As per last years cutoff, to get medical seat through All India Quota OBC category candidate was needed 560 and above score. So your chances are low to get government seat through AIQ but you have high chances to get government and semi government seat through state level quota.
