Neet is just an entrance examination.
Neet is a National level entrance examination and the single entrance examination by qualifying which you can get admission in all the medical colleges be it private or government including AIIMS and JIPMER for
MBBS
BDS
Ayush courses( Ayurveda i. e BAMS , Unani i.e B. U. M. S, Siddha i. e BSMS, and Homeopathy i.e BHMS ) and
veterinary i. e BVSc. ( for BVsc some states also have their own entrance like RPVT in Rajasthan for state quota seats, neet is mandatory only for all india quota bvsc seats offered through AIPVT counselling)

Here's how you can get admission in MBBS :-
----------------) Appear in Neet examination
To be eligible for neet one must be either appearing for 12th board examination 2021 or should have passed 12th board examination with physics, chemistry ,biology/biotechnology and English and must have got at least 50 % in aggregate of PCB i.e Physics, Chemistry and Biology /biotechnology in 12th examination if belongs to general category , or at least 40% in aggregate of PCB if belongs to Obc/ Sc /st category. ----------------) Qualify neet examination. Qualifying cut off in neet is given below
General :- 50th Percentile
OBC:- 40th Percentile
SC :-40th Percentile
ST :- 40th percentile
To be on the safer side, out of 720 marks, target at least around 580 -630 marks for state quota seats and 650 marks in neet for All india quota seats if you belong to general category.
----------------) Participate in neet counselling. There are 2 counsellings -
Mcc counselling :- It is conducted for 15% AIQ seats, 100% Deemed Universities seats, Central Universities seats (Delhi University, AMU & BHU ), 100% ESIC seats , AFMC seats (only Registration Part) & I.P University (VMMC & SJH AND ABVIMS & RML & ESIC Dental, Delhi (15% AIQ + 85% Institutional Quota ) ,100% AIIMS seats & 100% JIPMER seats

There is no need to submit document in NEET counselling which will be attested by a gazetted officer. Show your document which will be self attested by you. There is no compulsory rules for copy of document submitted at the Neet counselling will be attested by gazetted officer. If it is not asked specially for specific reasons to submit documents attested by gazetted officer there is no need to show your documents attested by gazetted officer.

Class 12th details have nothing to do do with neet examination apart from verification of marks. You can definitely use your caste certificate for getting the benefits of reservation in NEET UG. You would get the option to select your category while filling the NEET UG form as well as during the counselling session while registering for the same.

It's not required in AIQ admissions nor for State quota. There is no requirement while giving the exam.
Gap certificates are generally required for droppers just to prove that in the gap period the candidate must not took admission somewhere else .And if it's required it'll be required at the time of counselling.

Unfortunately on such NEET score you are not even qualified for NEET 2021 because the cut-offs for NEET UG 2021 are as follows :-
-->>> General - 50th percentile - 720-138 marks
-->>> SC/ST/OBC - 40th percentile - 137-108 marks
-->>> General-PwD - 45th percentile - 137-122 marks
-->>> SC/ST/OBC-PwD - 40th percentile - 121-108 marks
Now without getting qualified in NEET UG 2021 you cannot get admissions in any of the courses such as MBBS, BAMS, BHMS and other Ayush courses neither in government college nor in private college.
