Dear aspirant
You will have to provide caste certificate from the same state which you are applying for. You can't use up state EWS certificate in the Uttarakhand. You must obtain your EWS certificate from Uttarakhand to claim EWS reservation.
You can use central EWS certificate obtained from Uttar Pradesh in the AIQ counseling. Rather than that you will have to provide same state caste certificate for which you are providing the domicile certificate or the state in which you are taking admission.

You do not have to really worry about the medical fitness certificate as
The medical college you are going to get in counselling have its own teaching hospital
And you will have a medical check up there itself at the time of admission and do not worry it's just a formality unless you have really big issues such has handicapped , colour blindness , etc
And trust me , you will enjoy your check up as you will first time see your hospital , you will be treated as special guest , you will stand out of line , will be attended first and completely free , you will get a feel of a doctor
And in case you want to be too sure , you can get fitness certificate from any Gov. Hospital .

There are different counselling process of All India quota seats and state quota seats. The counselling for Deemed Universities is held through All India counselling. So to apply for deemed university college you have to register yourself for All India counselling.
But to get admission in medical colleges of your domicile state you need to apply for state quota counselling. There is no upgradation process. As both are two separate process. You can apply for both simultaneously.
Also if you get allotted in first round of any of the counselling then also also you can apply for their second rounds too.

If you get admission in deemed university allotted to you in first round after that also you can take part in 2nd round of counselling. It is valid and also mentioned in the guidelines issued during counselling. Say if you blocked on seat in deemed university and in some State counselling you get government seat in 2nd round so you can leave your deemed university seat and can take admission in government college.

And as per the previous year's cut-off trends you must obtain a rank around 10505 - 227870 for securing a BHMS seat in Bhartiya Vidyapeeth, Pune.
So this suggests your chances for securing a BHMS seat at Bhartiya Vidyapeeth are low .

The counselling is not started yet for Manipal University. Sikkim Manipal University MBBS counselling will be conducted in offline mode for UG medical admissions, therefore, candidates need to report to the college in person.

At the time of counseling at least 8 passport size photographs are required. Currently you have 6 photographs, so you can make more copies of the same photograph from your nearest photo studio, that you used in your NEET 2021 application and you will be alright.

In the Round 1 of All India Counselling, Free Exit is allowed. Free Exit means if a candidate is allotted a seat in this round and even if the candidate does not report/join the allotted college he would not be penalized and will be eligible for subsequent round. While in the Round 2 of All India Counselling free exit is not allowed. The candidate can exit the seat with forfeiture of security amount option.
