See EBC category is a reservation category for state level but NEET is a national level medical entrance examination and in such entrance examinations only central reservation categories will be applicable so if your category is not listed in the central list of reservation then you should opt for General category but you will get benefit of your reservation category i.e. EBC but during state quota counselling of Bihar as it is the state level reservation. But if you go for AIQ counselling then you will be allotted seat only through General category if your category is not listed on the central list of reservation.

Yes, during form filling, you just have to agree that you are an SC, no proof will be asked, but during counselling, certificate will be required and if you fail to do so, your admission will be done under General Category. Thanks

I am guessing that, your name is same on your 10th and 12th certificate and different only on your domicile and caste certificate
If your name is different on your domicile and caste certificate then you still have time, you can get new domicile and caste certificates made with correct name.
In case, if your name is different 10th on 12th certificate then you can get an affidavit done mentioning that both the name belongs to the same person i. e you
Also, one needs to fill name / parents name /dob etc in neet application form as per 10th certificate.

limited time is left for neet 2021 , so you should be focusing on your preparation,
practising chapterwise questions
then revising the chapters,
solve at least previous 10 year neet question papers as concept behind the question often gets repeated in the examination so it will be of great help in your preparation ,
go through neet mock test papers as it will let you know your preparation level and and weak areas so that you can work on them and perform well in your examination.

Yes as per the information bulletin you can appear for NEET 2021 even if you have Biology as your additional/optional subject. Because until the Hon'ble SC pass it's judgement the candidates having Biology/Bio-technolgy as an additional or optional subject can appear for NEET 2021.
But there is another condition that you must have passed/be passing your class 12th/PUC with minimum aggregate of 50% in PCB if you belongs to Unreserved category, 40% for OBC/SC/ST and 45% for PwD certificate holders. Also note that in your curriculum English subject should also be there.
Additional and optional means same in regards to the class 12th subjects so not to worry.
Also there is age limit so if your age is between 17 to 25 years before December 31st 2021 and also fulfil above criterion then you can appear for NEET UG 2021.

Generally there is no need to worry about it. Codes are usually given to know certain number of candidates appearing for NEET Entrance exam. For instance, number of candidates appearing in NEET who are also pursuing degree, or droppers, et cetera

ALL INDIA QUOTA is 15 % total. So in this as well there is reservation for all the category. OBC/ SC /ST separate quota is their for each of them. You dont need to worry for this.
Reservation is mandatory in both 85 % and 15 % quota. Hence, there is no doubt and confusion for 43 % pcb. For general category only, the requirement is 50 % in10 + 2 higher secondary from a recognized college / university.

I am guessing that you are not in the OBC NCL Central list but you belong to OBC NCL at state level
You are having misconception, even in state colleges there is OBC reservation in 15% all India counselling.
But one important thing to note down is that to avail OBC reservation in 15% all India counselling you must be in the central list of OBC NCL otherwise you will have to participate in counselling as General category.
And coming to seat allotment,
Yes, in 15% all india counselling if you are not in Central list of obc then you are eligible for seat allotment in general seats only and not seats reserved for obc .
And don't worry you will be able to select OBC NCL category during your state counselling and can avail reservation in state counselling.
