No, NEET candidates are not required to submit gap year certificates , neither at the time of All India Quota (AIQ) counselling or State Quota counselling
Gap certificate is only required for droppers, It's needed to make sure that you did not take any Addmission in any other course , or get involved in any criminal or law breaking activity during that period and if you have joined any college without taking any break then you have to submit migration certificate and leaving certificate from the concerned university during the time of admission instead of gap certificate

I regret to tell you this but IPU have released its application form for various courses including MBBS and 31st july is the last date to fill these forms , you still have 3.5 hours to fill the form right now , i would suggest please do this today as after NEET no application form will be released only councelling will be done in IPU

No, you will not face any problem. You have to score at least 50 % aggregate in NIOS board examination. i.e 10 + 2 boards.
According to latest verdict, NIOS candidates are valid to write NEET examinations. It is a recognized board and acceptable.
You can go ahead with your counseling for NEET exams.

Of course other state students can apply for UP state counseling but there's a fact i.e, you'll be only able to get admission in private medical colleges. You won't get seat in any of the government medical colleges.
The security deposit money is a sum of Rs. 30,000 which is to be paid who are applying for government medical colleges admission. Those who are applying for private medical colleges they need to pay a security money of Rs.2lakh.This security money is refunded once the admission procedure is completed.

Legally you should change your documents is the general rule. Now if you don't and you have to take admission then you have to provide a affidavit providing that you are providing all the information to be correct and the certificates, documents you provide are of the same i.e., you with two names as your new name and your old name. You can visit any lawyer and he/she can draft it.
I would highly recommend you to contact the college you want to take admission in and get it discussed with the officials. They may provide you more information regarding the format of affidavit or the rules you need to abide etc.

You will have to report to your alloted college after the counseling ends. You will be notified about that in advance and will be given a deadline date till which you will need to report to the alloted college in case you want to take admission in the respective college allotted to you.
Don't worry they will notify you about that. As far as your past records are concerned the NTA do keep records of every student about their attempts and marks scored
