To apply, visit the official website
Fill out the application form and make sure no compulsory section is left vacant
While filling up the application form the applicant needs to provide their Class 10th, 11th and 12th marks, Transfer Certificate, Proof of study from Class 6 to 12 and Community Certificate

As pharm-d is a doctorate degree one must score high I met probably and eligibility with 55% marks in 10+2

Both of the courses you have mentioned are equally good. BPT means Bachelors in Pharmacy and Pharma D is Doctorate in Medicine. If you want to be financially independent early then you can go with BPT as this course is popular these days and also has a great future too. On the other hand, medicine is also a part of our life and will be. It depends upon your interests.
You can start your work by just doing BPT but if you pursue MPT then the job opportunities also increase to an extent and it is also profitable for your knowledge as well as for your opportunities.

Different colleges have different policy of discontinuation. You can better contact the concerned authorities regarding the issue. However, it is completely legal and acceptable to discontinue a course if you wish to do so and collect all the concerned higher secondary mark sheets and certificates.
There isn't any eligibility rank for EAMCET. You need to qualify in EAMCET to be in the rank list. If you belong to general category you need to score minimum 25% marks out of 160 marks that is you need to score minimum 40%. If you belong to reserved category you need there is no qualifying mark, if you score a non zero positive score you will be allotted.
Seat allotment is based on category of the candidate, cut off rank, preferences of college and total number of seats available. The cut off rank is different for each category and college. SO you should aim to secure a decent rank. Analyzing the previous year cut off you need to score minimum 70-80 marks or above if you belong to SC/ST category, if you belong to EWS/OBC category you need to score above 80-90 marks or above and if you belong to general category you need to score minimum 90-100 marks or above to secure a decent rank to get seat in PHARM D.
