Filing of incorrect information will surely leads to failure of the examination form. There are very high chances that your application will be rejected but for that you must be updated incase of correction window open then you can edit the same.

National Testing Agency (NTA) has renamed UPSEE as UPCET (Uttar Pradesh Combined Entrance Test). The authorities will soon release the UPCET 2022 application form in online mode at the official website. The exact date for the release of the application form is not yet declared. CUET 2022 registration last date was May 22, 2022. Now there is no way you can apply for CUET 2022. The National Testing Agency (NTA) has re-opened the CUET 2022 application window on May 27 and it will be closed on May 31.State and private universities have been requested to consider the CUET 2022 results for UG admissions by the University Grants Commission (UGC).CUET 2022 registration form has been made available in online mode only. NTA has begun the CUET registration 2022 from April 6 at cuet.samarth.ac.in.

If the mobile number is already registered then in that case you must have registered in the portal already so try to login using the most common passwords that you use for registering on any website . Maybe you have registered and then forgotten . So try doing that .
If this does not work then either write a mail to ICAI regarding this problem or there are some helpline numbers available on the website that you can use to contact icai. You can also visit the nearest icai branch and discuss your problem there.

You haven't mentioned your category, I'll assume that you are Gen category students. Getting a govt medical College at 550 marks in Uttar Pradesh state quota counseling is very tough in Gen category.
In neet 2020 final cut off for Gen category students was AIR 16646 and 607 marks. in 2021 second round cut off for Gen category students was AIR 18298 and 601 marks.
So if you belong to Gen category and want a govt medical College seat through up state counseling you should must aim to score 600+ marks.
As you are in last week of preparation,try to solve more and more questions and revise the concept and formula and be confident in exam.
