I want to inform you that if you have the certificate issued from the central government or any equivalent authority then you can apply it for the NEET examination.
You can use your domicile for your state quota it will be applicable only in your state.
Be sure about the date of issue of all the certificates, as all the examination conducting authority provides a particular date for the certificates

As you have mentioned you have made a domicile certificate for neet UG 2021 and it has been verified on the e-district UP website but as per the guidelines you need to follow that.
Domicile certificate will always be accepted if it a recent one. And yours as you have mentioned that you have made your domicile certificate in 2016 so I would recommend you to make a recent domicile certificate to be on a safer side.

Bachelor of Physiotherapy (BPT)
Duration- 4.5 years( including 6 months compulsory internship)
Total Seats- 60 Total Fees- Rs 1.06 lakhs
Eligibility- Minimum of 50% marks required in 12 standard(PCB)
Selection criteria- Admission is done based on a valid score in UPUMS common entrance test followed by counselling.

Yes you are eligible for UP state quota and can avail OBC reservation as well.
This is because,
To be eligible for UP state quota it is not mandatory to have studied 10 or 12 from UP.
You need to fulfil any one of the following in order to be domicile of UP and thus be eligible for UP state quota :-
You must be native of UP and your father should be a Resident of the state of UP.
Your parent/s should have lived UP for at least 3 years.
So, as you are native/ domicile domicile of u.p. and have domicile certificate as well so you are eligible for UP state quota
Your parent/s should be working with the Government of India.
If you have passed your 10th and 12th from the state of UP, then you don't need to furnish domicile certificate and you will be eligible for UP state quota
If you have have passed either your 10th or 12th from UP, then you need to furnish UP residence proof of your parent/s in order to be eligible for UP state quota
So, if you satisy anyone of the above points then you will be eligible for UP state quota

See whether you apply using caste certificate of your state or of central level you will be able to take part in AIQ counselling as well as state quota counselling.
Now if you take part in state quota counselling then you can opt for more than one state to attend the state quota counselling of different states but there are some conditions which you should check before registering for counselling of that state. In state quota counselling two type of seats are allotted, government quota and management quota, for most of the states government quota seats are reserved for the candidates having domicile of their own state, other state candidates cannot participate in counselling for those seats but yes other state students can go for management quota seats.
Also for management quota seats fee is quite high as compared to that of government quota seats.

The home state quota or domicile reservation is usually availed by students who are permanent residents of the state.

See if you belongs to any other state, let's say Delhi then you will have to upload the category certificate of Delhi it does not matter where you are staying currently and as far as the matter of center, so center will be chosen by you and it has nothing to do with your category certificate so for e.g. if you are living currently in UP then you can opt for any center in UP and the center will be allotted to you in UP as per the availability of seats.
So don't worry about your category because reservation will be given to you as per the norms of your state and your Home state quota will not get change from the examination center.
So if you have domicile certificate of Delhi then you will be definitely eligible for reservation benefits of 85% Home state quota of Delhi.
Note that Delhi is taken as example for your state.

But your chances of getting a seat in YMCA Faridabad is a bit low because closing rank for YMCA Faridabad is 60031 as per previous year cut off . But still you can try for special rounds since there are chances for your admission in YMCA .Since you have up domicile therefore you will get home state quota in up colleges .
