Surname is not specifically asked in the application form, however in the guidelines it's clearly asked to fill the name as in your adhaar card. So, it's obvious you should put your surname in the application form.
If by mistake you filled the wrong details, you can edit them once the application reopens for correction. Even than if it's left unchanged no need to worry, you can always verify your documents using id proofs at the time of admissions in the college itself.
