With a NEET rank of 4,00,000 (General category), MBBS admission in government colleges is not possible. Private medical colleges also close much earlier. You may, however, get options in BDS, AYUSH courses, or high-fee private/deemed universities. Chances for MBBS are very low at this rank.

Honestly, with a NEET rank of around 4,00,000 in general category, it’s quite tough to get a government medical college seat. Generally, government MBBS seats close much earlier. At this rank, your chances are better in private medical colleges, deemed universities, or through state quota seats if cutoff goes a bit lower in your state. Still, competition is very high. You should also keep options like BDS, BAMS, BHMS, Veterinary, Nursing, or paramedical courses as backup. Sometimes students also go for private colleges if budget allows. I would suggest you to participate in counselling and fill all possible options, because sometimes cutoffs fluctuate. Keep an eye on your state’s counselling process too, as it may give you more clarity.
