Yes, you need to submit the affidavit for single girl child while filling the application form for NEET 2021. The option will be available, if you choose gender "Female" and you do not have any siblings(brothers/sisters). Kindly note that you nees to submit the affidavit in the format as mentioned by the NTA. You can check out the format here:
