It is always suggested to remain a little more careful while doing the paper work during admissions.
If Delhi Technological University has given a prescribed format of PWD certificate, it would be advised to you to follow that pattern in order to avoid any inconvenience during the admission process.
For a safer side, you could also make a PwD certificate in a format which suits you but don't forget the one prescribed by DTU for admission.
