Dear Shubhs, you should be using the name that is on the other official documents so as to let the process be hassle free. Ideally that is what will be printed on the admission papers as well. But if you want your Married name added on to your name and the PG documents, please get an affidavit done and it will serve the purpose.

I have updated the name in my medical council registration..and have a gazette copy of the same.. Do I need an affidavit too? Thanks a lot
