There is a proforma prescribed by NTA for NEET and it is available in the information brochure of NEET 2021 which you can access from the link given below :-

When you will open the above link you will find a PDF and after opening it you have to directly go to Appendix XV which is available on page number 108 of that PDF and it is the proforma which is prescribed by NTA for OBC NCL CATEGORY so you can check it out from there.
