You may contact your state liaison office however it entirely depends on the kind of mistake you have made in the form. You may get their contact information from your state's SCERT site. Ypu ,ay also get thir contact information from https://school.careers360.com/articles/ntse-application-form . I hope they can sort out the problem. All the best!
