Hello student,
To be eligible for UPSC, One should be 21 years of age and should have a bachelors degree or in the final year of graduation. But, you can start your preparation Now itself. Since you are in Delhi, you have access to the best ias institutes. You have two ways of preparation - one is self study and the other is to join any institute. But make sure you are ready to spend your time whole heartedly for the preparation. About your optional paper, you can choose medical science of you wish.
Good luck
