As per your marks you have a chance of getting admission in Govt Medical College Khandwa and Government Medical College Ratlam as previous year cutoff for government Medical College was 493 marks and for Government Medical College Ratlam was 495 marks under state quota so there is a chance that you get admission in government medical colleges although the probability of getting admitted is a bit low .
