As you have completed Bachelor's in physiotherapy i.e you don't have physics, chemistry and biology in your graduation , so you should choose code 02 dear on the basis of your 10+2 qualification as code 02 is for those who have qualified Class 12 in the year 2019 or before .

But you are eligible for Neet one if you have passed in Physics, Chemistry, Biology/Biotechnology and English individually in your 12th and must have obtained a minimum of 50% marks taken together in Physics, Chemistry and Biology/Bio-technology in your class 12th board examination from a recognized board if you belong to general category ; while it's only 40% aggregate in pcb for obc, sc and st categories , 45 % aggregate in pcb for pwd category.
