Hey dear, cutoff changes year to year, if you see the last year's cutoff which went very high that even students getting 500 marks were unable to get a good govt. Mbbs seat. So, with 486 marks being an obc candidate you have some chances of mbbs under state quota if the cutoff falls less than previous year, otherwise you may also go for bams and bhms. You can also use college predictor through this link:

The cutoff marks changes every year. It depends upon number of candidates that appeared in examination, number of seat available in colleges, difficulties of question paper etc. So as per expected cutoff marks you have chance to get government medical college through state quota if you belong to OBC catagory. Or you can easily get a seat on any semi government colleges.
You can predict your college with neet college predictor which helps you to know which is you may get.
