As per neet 2021 information brochure

Your photograph should preferably mention  your name and the date on which the photograph was clicked. So name and date on photo is not mandatory but  definitely preferable.

And ,

when it comes to left hand thumb impression then it should be with blue ink but you have used black ink.

But the colour of ink shouldn't be the reason for rejection of your application form .

Any way ,

NTA doesn't reject your application form a straight away, it does notify you if there is any issue with your images uploaded by you through the given email ID so if there are any issues they will contact you just make sure that you to keep the emai ID with which you have registered for Neet 2021 and then they will allow you to change your photo uploaded by you and upload it  as per the specifications . Moreover, correction window is also opened and if they allow image correction then you can change your thumb impression and photograph without waiting for NTA mail or notification.
