its fine that you are average student. The student which are getting good marks practice alot for which they get marks.
so you have give atleast your 16 hours daily to your Physics,chemistry and mathematics
you can go for book
1.Concepts of Physics by HC Verma both 1st year and 2nd year book it has good problems try to solve them yourself.
2.For chemistry, you must be having books your intermediate chemistry books. To practice you can use OP Tondon book. It's a bulky one but you have to. It comes in 3 different part for .i.e. physical chemistry, organic chemistry, inorganic chemistry.
3. For maths go for topics and solve the problems given there also the elements. from your intermediate

to score high you have to work hard. Book will only give you concepts.
