In Aiims final registration I cleared 12th in 2018 then should I add theory and practical marks total or theory only ???

you need to fill the total marks or the sum of both theory and practical in the aiims application form.
if you are not able to find then check it on your 12th board certificate. you must need a gap year certificate but that will be helpful in future in your admission process.
