In Fddi AIST 2019,I got 21 rank for MBA program. My question is Medical fitness certificate or physical fitness certificate is same or not

Hope you are doing well.
Answering to your question, Yes both Medical Fitness Certificate and Physical Fitness Certificate are the same and hold the same value.
Both have to be issued from a legal practitioner or Govt. Hospital.
