Hello Ram, Josaa has a standard medical certificate format for such cases of physically handicap candidates. You can find the pdf in this link: https://josaa.nic.in/webinfocms/Public/View.aspx?page=73

make sure along with the duly filled certificate you also carry your PwD certificate issued by the state govt/ward of your home state.
