With an AIR of around 1.2 lakh and a general category rank of 41k, your chances of getting an MBBS seat in Delhi are very low, as government medical colleges in Delhi under state quota usually close around AIR 10k to 20k for general candidates. Even in private/minority institutes, the cutoffs are quite high.

In Madhya Pradesh, however, you have a better shot. State quota MBBS seats in newer or less preferred government colleges in MP have closed around AIR 1.05 lakh to 1.25 lakh in previous years. So with your rank, you are on the borderline and may get a seat in later rounds or mop-up round, especially in colleges like Shahdol, Khandwa, Datia, or Chhindwara.

You should definitely register for MP State NEET UG counselling and keep all government colleges in your choice list. Participating in mop-up and stray rounds is also important, as ranks do slide there.
