Dear Student,

According to my knowledge , there are total 5 pages in NEET admit card this time.

NEET admit card consists of the following :-

1. Name of the candidate

2. Fathers name

3. Date of birth

4. Gender

5. Application number

6. Roll number

7. QR code

8. Category of applicant

9. Sub-category

10. NEET 2021 exam date and time

11. Reporting time and time for last entry

12. Address of aspirant

13. Language (medium) of question paper

14. NEET exam centre name and address

15. Exam centre number

16. Passport size photograph (uploaded in the application form)

17. Signature (uploaded during registration)

18. The IP address and date of downloading of admit card

19. Space to paste passport size photograph (same as used during registration)

20. Signature of the candidate (to be done in the exam hall in front of the invigilator)

21. Signature of parent

22. NEET UG senior director signature

23. Other general instructions to be followed

With NEET admit card 2021, you must carry the below mentioned documents :-

  • Passport size photograph (same as on the application form)
  • Valid ID proof [ PAN card/Driving license/Voter ID /Passport/Aadhaar Card (with photograph) /Ration card any other government document ]
  • PwD certificate (if applicable)

Hello aspirant,

This Admit Card and related details are in four pages.
  • Page 1 contains the Centre details and Self Declaration (Undertaking) form regarding COVID-19.
  • Page 2 has Important instruction for candidates
  • Page 3 and 4 have Advisory for candidates regarding COVID-19

You don't need to worry if your 2 pages are blank, you have to carry the pages has admit card with self declaration only.

Candidates are advised to carry only the following items with them into the examination venue:

  • Admit Card along with Self Declaration (Undertaking) downloaded from the NTA website (a clear printout on A4 size paper) duly filled in.
  • A simple transparent Ball Point Pen
  • Additional photograph, to be pasted on attendance sheet
  • Personal hand sanitizer (50 ml)
  • Personal transparent water bottle
  • Valid ID proof (PAN card/Driving License/Voter ID/12th Class Board Admit or Registration card/ Passport/ Aadhaar Card (With photograph)/ E-Aadhaar/ Ration Card/ Aadhaar Enrolment No. with Photo)
