Dear Anurag, on the official website, to the right bottom, you will find a heading called, Certificate. Under that you will find EWS Certificate in a downloadable pdf format. Please download that, take a print out, fill it up and self attest it. And get it scanned and upload it as a document for Category Certificate. That is the procedure to be followed.
