If you have applied through the PWD quota for the NEET counselling then you will have to generate the certificate in order to get the PWD quota seats. For issuing the certificate you will have to go through the medical checkup through the medical assessment centers mentioned below.

• All India Institute of Physcial Medicine and Rehabilitation, Hazi AliPark, K. Khadya Marg, Mahalaxmi, Mumbai
• Institute of Post Graduate Medical Education & Research, 244Archarya J.C. Bose Marg, Kolkata
• Madras Medical College, Park Town, Chennai

You can go to these centers for the medical check up and then you will get the certificate for the counselling.
