As per as your score and previous year cuttoff score is medical colleges of kerela,you seems to have less chance of getting government medical college.You need 560+ score for government medical college.You can secure seats in private self financing medical college.I am providing you link of previous year cuttoff of various medical college of kerela:

what is the procedure of admission to self financing collages like st.johns ,kasturba mc and will the fee be according to mark/rank secured
