• NEET rank letter
• Birth certificate 
• 10th class certificate 
• 12th class certificate 
• 12th class marksheet
• 8 passport photos of yourself
• Proof of identity 
• Category certificate 
• Provisional allotment letter from online

You have to take the original copies of all the documents along with self attested photocopies.

Dear Sir/ Ma'm, during the state counselling of UP, you need to carry your NEET admit card, the rank letter of NEET, burth certificate, Class 10 and class 12 marksheet, 8 passport sized photographs, identity proof, provisional allotment letter and category proof documents(if applicable). Except the photographs, you are advised to take 2 zerox copies of each document.
