You can go to NOTARY office nearby and tell them to issue a gap year certificate and tell then the reason behind taking an year gap .
You will get your affidavit soon .

The institutes who are asking for Gap Year Affidavit would be having Format of that on their official websites. And if the Counselling is for group of colleges then the official website for that would be having its format. If you get the format then visit to Notary Office (Advocate) and provide them information the same.
Still if you are not able to find then contact them via Phone Call or E-Mail for the same.
