what i should do as i did not mention name and date in photo scanned in neet ug form

Not mentioning Name and Date doesn't call for an application rejection, Megha. Please wait to hear from NTA for any notification. If NTA wants the uploaded photograph to be changed, NTA will notify you through your registered Email Id and mobile no# and then you can make the necessary changes.
