Medical certificate is a mandatory requirement for counselling in some institutions. It is required after the counselling starts.
It is usually one of the many documents that need to be submitted for admission.
The format will be provided by the institution itself. It completely and wholly depends on the institution you are applying for.
For example, JoSAA requires the submission of medical certificate only after the counselling starts and it provides the format too.

It is required at the time of counselling. And the certificate is given by the hospital.You only need to tell him about the certificate he will check and provide you that.

Any registered Doctor (A Legal Physician) and that doctor will do the necessarY

Just take the printout of page 53(Annexure 8) from business rules pdf from josaa website and take the print out.

Go to any government hospital and ask them for check ups written in format of medical certificate in JOSAA's BUSINESS RULES.

Plz note that you have to go only in gov hospital otherwise your certificate would be invalid.

You can go to a private clinic but first ask the doctor if he/she has a valid medical registration number and if he/ has a registered clinic if which seal can be used for filling of the medical certificate……
