No, there is no mention of such requirement. You need not have the name and date printed on the photograph. The requirements of the photograph to be uploaded are stated below:

1) The photograph should be in JPEG/JPG file format with dimension 3.5 cm x 4.5 cm. 

2) The size of the file should be between 50 kb to 100 kb. 

3) The photograph must be colour or black and white with clear contrast.
