It is calculated by how many students are behind your marks or you can say, if your percentile is 97, it can be assumed that you are among the top 3% students of those who have beemln qualified in the examination .

Your All India and category wise percentile will be known to with your results in your NEET scorecard. Percentile indicated the percentage of people you have done better than. The cutoff for qualification is getting above the 50th percentile (general category) and 40th percentile (SC/ST/OBC).
