If a work-life balance is most important to you, then Ophthalmology is the better option; if your primary goal is to work in a surgical field and you love challenges, then ENT makes more sense; and if you have no problems with flexible hours and have open interests with getting in to high demand specialties, then Anesthesia will provide you a consistent and stable scope of practice. All in all, I think you should seriously consider which field you could see yourself practicing for a long time, not just because of your current medical rank.
