At first congratulations for your result and best Wishes for your future. You must go for both counselling for All India quota as well as state quota also. I am afraid to say with this marks your chances to get government colleges are almost negative through All India counselling for 15% seats . You can still try in state counselling for remaining 85% seats . Different state counselling cut offs vary from each other. For different states it depend upon availability of colleges, student number etc various things . You have not mentioned your state . You must mention that for us to answer precisely. You must go for your respective state's counselling. Though chances for you are tough for you in there also for government colleges. However you can get easily many private medical colleges. As you have mentioned you want to change your category just before payment so I do not think that is possible in any case. So, when they ask for document verification if you are unable to submit your certificate as a proof against your mentioned category then you would be treated as a general category candidate . But in your case I am not sure that is possible and you have to proceed with your chosen category initially.

As per the information available on the official website of DMC Ludhiana, it does not take part in All India counselling. The college offers admission for 100 seats every year, which is distributed for g overnment Quota - 50 and Management Quota : 50 (Including NRI category 15 Seats). Admissions to DMCH are done by the B. Farid University of Health Sciences, Faridkot, which is the main state counselling authority in Punjab.
We should wait at least 24 hours for conformation of payment or for refund of the required amount if it was not successful. The updation will take 24 hours maximum. If the confirmation page is still not generated and its showing not successful, pay the fees once again and take a printout of the confirmation page. The excess fee paid will be automatically refunded in the account from which application fee has been remitted. If the problem is still not resolved, contact the respective banks for further clarification.

According to this website, the minimum qualifying marks required to get chance for OBC category student in this prestigious institution in 2019 was 373 where your marks is far behind from it. I am afraid that you cannot change your category now. After the application forms have been filled and the results being declared, it is already late to change the category. Since you have chosen OBC category, you will have to produce OBC caste certificate for validation and verification of documents. If you fail to do so, your admission might get cancelled.

If you get a seat in second round of all India Counselling and you do not join the college, then you can exit the all India counselling with forfeiture of security fees. But if you report to the allotted college in second round of counselling, then you cannot leave the seat and go to state counselling.
If you exit the second round counselling, then you have to register fresh during the mop up round of all India counselling if you want to participate in it.
If you get a seat in second round of state counselling, then you have to report to the allotted college and further agree to participate in further rounds.

At a score of 508, unless you belong to some reserved category chances look bleak for you to secure a seat in any Government or State Medical College, especially Important Medical Institutes like GMC Kannur. However, do keep a check on the Government and State Medical Counselling Dates and Procedure.
Secondly, if you do not have any issues, you might consider a drop this year. As far as I have seen students securing in 450-550 range in first try, minimum increase their score to 600-650 range in second try, and get assured of a seat in a National Medical College of Choice.

OBC has two categories namely, Creamy layer and non-creamy layer. In case, the family income is less than 8 lakh per annum then, the candidate is considered as non-creamy layer. In your case, the income is more than 8 lakh, you are considered as creamy layer and would not get any reservations.
You are still eligible for All India Quota counselling under unreserved category. If you are willing to avail the reservation then, you can surely apply for State Quota Counselling.
