As you have mentioned that you have got 38k rank in viteee 2021 and you want to take up biomedical engineering in category 1 so let me tell you as per my knowledge that you have good chances to get there in category 1.
But these things depends on certain factors like the number of candidates applying for the branch, number of seats vacant . But at the worst case you will definitely get in category 2

Previous years records says that the candidates getting less than 20,000 rank are considered under category1 and 20k onwards category2 starts and so on. So there are decent chances that you might come under category-2 or category-3. But there is no chance for category-1. And as per the last year cutoff trends, you might get Civil engineering at VIT Vellore and Chennai campus, Production and Industrial Engg at VIT Vellore, BioMedical Engineering at VIT Vellore,Electronics and Communication Engineering at VIT Chennai, Electrical and Electronics Engineering at VIT Chennai, Mechanic engg at VIT Chennai, Electronics and Computer Engineering at VIT Chennai and Fashin Technology at VIT Chennai.

BSC nursing and biomedical engineering both are different courses.
BSC nursing is 4 years degree course it's actually a noble profession. You will be served for patients for world. In this course you have to learn every study about the nursing like anatomy, opt handling, physiology etc.
Biomedical engineering is basically a department of engineering course. It's 4 years degree course. But it's mainly the machine related course which in important for medical uses. So you will be part of a medical line also.
Now it's totally depends on you which department you want to choose.
