Voter card/Aadhar card/Passport of parent/s in case of domicile certificate of parent.

PwD certificate, if applicable

Well usually based on previous years cutoffs, for BPT course to a get a government seat , a minimum would be 250-280+ and this year it may go up. So your chances for a government college is low but you may try for private colleges . I see you are from Maharashtra so some colleges you may try are :

Firstly congrats for such a good score. As per last years cutoff your score in NEET is good enough to get a government medical seat in Andhra Pradesh through quota. It seems difficult for you to get a medical seat through 15% All India Quota but there are chances to get government medical seat through state quota.

if the seats are not vacant in ews category then your are not eligible to take admission But you can take open admissions if and only if your chosen college has management quota or direct admission feature.

Your marks is quite good but due to increase in cut off your rank has been shifted a bit far from the last year cut off of KGMU, Lucknow. There was not reservation for EWS category last year in KGMU, Lucknow.
The general category rank was around 1900. There might be a possibility that they will include EWS seats in their admission from this year and you may get a seat.

It does not matter whether you have an EWS certificate issued by the State Government or Central Government. As long as you have a valid EWS certificate with a valid date issued, you can attend the counseling without any fear.
