I hope you are doing good.
MBBS seats are only given on the basis of NEET scores. With 400 marks, it's little bit tough to get MBBS. I would suggest to go for bds,bams or bhms courses. In sha Allah everything will happen according to your plan. Hope for positive.

Yes if you have given neet exam than it is compulsory to fill the neet phase 2 registration form after filling that your result will be declared and it is necessary if you wish to take admission in Medical colleges.
In the second phase of NEET 2021 application form, candidates need to fill their personal, class 10 and 11, parent income details and upload the documents as per the specifications.
